Job Summary

The successful candidate will be responsible for planning and managing patient care according to individual needs. This includes interviewing patients to record their medical history and physical condition, obtaining vital signs, and providing routine care.

Key Responsibilities
  • Provide 1:1 care for clients, following established plans of care while maintaining dignity and independence.
  • Monitor clients' vital statistics and report abnormal findings to case managers and physicians.
  • Administer medications, feedings, oxygen, and ostomy care as needed, per client's plan of care, and document results.
  • Complete routines established by other care professionals, such as Physical Therapists, Occupational Therapists, and Speech Therapists.
  • Ensure infection control policies are maintained at all times, reporting possible communicable diseases or infections, and implementing infection control procedures.
  • Accurately document nursing actions and communicate with family, case managers, and physicians regarding progress and outcomes.
  • Order supplies and medications as needed to ensure adequate amounts are on hand.
  • Monitor incidents involving individuals, complete incident reports, and inform case managers of any changes in a client's medical condition or needs.
  • Ensure all medical equipment is properly maintained and report faulty equipment to Durable Medical Equipment for repair.
Requirements
  • Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
  • Knowledge of individual service plans, including understanding of client rights and health needs.
  • Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
  • CPR Certification.
  • Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
  • In-depth understanding of patient care policies and regulations.
  • Familiarity with symptoms and signs of improvement for several medical conditions.
  • May require moderate travel between facilities.
