Job Overview:

As a Registered Nurse at Personal Touch Home Care, you will play a crucial role in planning, organizing, and delivering home care services. We seek experienced nurses who are passionate about community health education and engagement.

By utilizing community resources, you will customize services to address the unique needs of individuals and families in their homes.

Responsibilities:
  • Conduct thorough assessments of patients and families to identify home care needs.
  • Provide comprehensive physical assessments and medical histories.
  • Continuously evaluate patient needs and modify care plans as necessary.
  • Collaborate with physicians to create personalized care plans.
  • Engage patients and families in the planning process.
  • Implement preventive and rehabilitative nursing procedures.
  • Administer medications and treatments as prescribed.
  • Offer counseling and health education to patients and families.
  • Identify discharge planning needs and implement plans prior to discharge.
  • Perform peripheral short intravenous line establishment and administer IV medications under physician orders and with demonstrated competency.
Qualifications:
  • Current licensure in the State of Virginia.
  • CPR certification and a valid driver's license.
  • Preferably, a minimum of one year of recent acute or long-term care experience.
  • Bachelor's degree preferred, with at least one year of experience in home care.
  • While management experience is not required, you will supervise Home Health Aides and Personal Care Aides.
  • Strong observation, verbal, and written communication skills.
  • Excellent problem-solving abilities and basic math skills.
