The role covers the northern suburbs including Maywood, Oak Park, Forest Park, Melrose Park, Elmwood Park, Franklin Park, Schiller Park, Schaumburg, Carol Stream, and Chicago zip codes north of North Avenue. You will provide one-on-one home-based physical therapy, collaborating with physicians, nurses, social workers, and home health aides to deliver the highest standard of care. Minimum Qualifications Education: Bachelor\'s degree with progressive experience or a Master\'s degree in Physical Therapy. Licensure: Must be licensed to practice as a Physical Therapist in Illinois. Experience: Previous experience in home healthcare or familiarity with conditions of participation is preferred. Additional: Must have a current driver\'s license and reliable transportation to travel to and from patient sites.
