We are seeking an experienced NURSE ASSESSMENT COORDINATOR (NAC) to join our team 

THE RESPONSIBILITIES FOR THE  NURSE ASSESSMENT COORDINATOR (NAC) POSITION INCLUDE:

  • Responsible for the accurate and timely completion of all Medicare/Medicaid case documents in order to assure appropriate reimbursement for care and services provided within the facility.
  • Conducts current Minimum Data Set (MDS) reviews to assure achievement of optimal allowable reimbursement under the current Patient Driven Payment Method.
  • Oversees the overall process and tracking of MDS/PDPM documentation and submission.
  • Integrate nursing, dietitian, therapeutic recreation, restorative, rehabilitation, and physician services to ensure appropriate, accurate assessments which capture all clinical conditions and provided services.
  • Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay.
  • Responsible for electronic submission of all MDS assessments. Verifies electronic submission of MDS and performs corrections when necessary and maintains appropriate records.
  • Assists disciplines with care planning process by ensuring the MDS information is completed timely, the Care Area Assessments (CAAs) are analyzed thoroughly, and the team has completed the MDS prior to care conference.
  • Ensure all Medicare documentation follows federal and state guidelines and is completed timely. This includes the physician certification/Recertification, Traditional Medicare and Medicare Advantage non-coverage letters for part A and B.
  • Assess and evaluates the outcomes of the RAI process to determine additional training, education and monitoring needs
  • Actively participates in Clinical Quality Initiatives including understanding the role of MDS accuracy in Quality Measures, Quality Reporting Program (QRP) and facility 5-Star Rating.
  • Provide survey support during state and/or federal Nursing Facility surveys

QUALIFICATIONS FOR THE NURSE ASSESSMENT COORDINATOR (NAC) POSITION:

  • Active RN or LPN license, preferably in the state of Delaware.
  • 1-3 years of experience as an MDS Coordinator or equivalent experience
