This position is also responsible for coordinating and facilitating activities specific to Memory Care Residents in collaboration with the Interdisciplinary Team. The Memory Care Support CNA provides patient and family-centered care consistent with Agrace's philosophy, goals, standards, regulations, and policies.Assist Patients with Activities of Daily LivingAssist with ambulation, changing position, and moving to/from sitting and standing.Help with bathroom functions like toileting and incontinence care.Help bathe, shower, dress, provide oral care, and groom.Assist with eating and drinking.Perform post mortem care.Obtain vital signs. Assist with treatments.Anticipate and respond to patient needs and round on patients.Provide homemaker assistance Perform light housekeeping such as changing linens, doing laundry, tidying room, and removing trash.Deliver patient supplies.Deliver food and beverages.Communication Read, understand, and follow the plan of care.
