POSITION SUMMARY

The Mobile Assessor / Family Counselor is responsible for providing assessment services for those clients who are seeking outpatient treatment services and who find one of our IOP offices to be a convenient location. The Mobile Assessor will work closely with the referral sources in coordinating a seamless transition of care for clients needing further treatment. The Mobile Assessor / Family Counselor will also be responsible for performing assessments in a hospital or clinical setting as needed. This position will also present the various programs of Cumberland Heights to area schools, hospitals, physicians and counselors in private practice. The IOP Mobile Assessor is responsible for performing initial screening, assessment, pre-certification when indicated and completion of all paperwork for potential clients and serves as a liaison between the patient and the admissions staff. As family counselor, this employee provides psycho-education and structured group and individual counseling and/or therapy to patients and/or families impacted by substance use disorders. The Mobile Assessor / Family Counselor also covers groups in the absence of the primary counselor. This position coordinates care with IOP primary counselors and Directors and is part of the IOP treatment Team.

PRIMARY DUTIES AND RESPONSIBILITIES include some or all of the following:

Screens inquiry calls and conducts face to face chemical addiction assessments to determine appropriateness for admission, within scope of practice as defined by licensure and clinical privileges.
Provides provisional diagnoses based on assessment findings within scope of practice as defined by licensure and clinical privileges.
Communicates clearly with each prospective patient their treatment options and, to the extent possible, their probable financial responsibility.
Meets scheduled appointments in a timely and professional manner.
Determines appropriate level of care for each person assessed for services within the scope of Cumberland Heights programs, or makes an appropriate documented referral for persons who would best be served in a different program.
Communicates and cooperates with admissions, benefits verification and UR personnel and provides timely and complete documentation of the admission to UR and data entry personnel.
Coordinates clinical consultation with the appropriate Clinical Manager regarding the admission appropriateness of persons who may have co-occurring psychiatric disorders or medical instability.
Documents clinical activities in patient record according to established policies and procedures.
Ensures that potential patients and referral sources are given accurate and complete information about the services offered by Cumberland Heights, and when possible, introduces potential patient to the care provider with whom the patient will be working.
Facilitates individual, group and family counseling and/or therapy sessions, within scope of practice.
Assesses clinical problems and develops treatment plan, in conjunction with the patient.
Participates in daily and weekly team meetings and treatment plan review.
Provides educational components relative to addiction and recovery to patients and families.
Communicates regularly with family members, Employee Assistance Professionals, physicians, referrals, judges, probation officers, etc. regarding treatment progress and continuing care needs.
Recommends ways to improve the quality and delivery of services.
Maintains confidentiality of company and patient information.
Reacts productively to change

Requirements:

QUALIFICATIONS

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. They will demonstrate the ability to work independently, have extensive knowledge of the services offered by Cumberland Heights and consistently demonstrate excellent clinical skills/critical thinking ability. They also will have extensive knowledge of chemical dependency and the disease concept of addiction. The ability to work with third-party payors is essential in order to perform precertification for potential clients. The requirements listed below are representative of the knowledge, skills and/or abilities required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

EDUCATION AND/OR EXPERIENCE

LADAC, LPC, LMSW, LCSW or Master’s Degree in Counseling or related field required; five years experience in behavioral healthcare with addictions specific experience preferred or other job related activities.

OTHER SKILLS

Ability to lift 30 lbs; ability to speak, hear, see, sit, walk and stand; ability to speak, read and write in English; excellent customer service and interpersonal skills; good problem-solving and analytical skills; excellent written and oral communication skills; computer keyboard skills with ability to quickly learn and use clinical software; knowledge of signs/symptoms of intoxication/withdrawal of alcohol and common drugs of abuse; knowledge of general options for addiction treatment; excellent interviewing skills; excellent observational skills; leadership skills; ability to work calmly, professionally and as an ally with people in crisis; crisis prevention certified; knowledge of and ability to apply information regarding age/cultural/population specific characteristics to patient care in assigned area. If recovering, two years of verifiable abstinence required with five years preferred; active participation in the appropriate Twelve Step program preferred.

WORK ENVIRONMENT

Position is in the community and will include different settings that involve everyday risks or discomforts requiring normal safety precautions. Position requires frequent contact with people in crisis, including those who are chemically impaired and/or volatile and may require additional safety precautions. Position is an on-call position and subject to varying hours, including evenings and weekends.
