*QUALIFICATIONS for the Registered Nurse (RN) Case Manager*
* Graduate of state-approved school with course in pharmacology
* Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state.
*Job Functions:*
* Act as primary liaison between physician, patient/family and home health team.
* Provide direct patient care services to patients and supportive care to the patients’ families.
* Manage patient case load.
* Building a trusting and compassionate relationship with your patients and their families.
* Empower patients’ caregivers by educating them on care for their loved one.
* Manage and collaborate with an interdisciplinary team.
* Evaluate patient and family needs, and assist in the preparation and execution of the plan of care for each patient.
* Maintain all assigned patient clinical records.
* Participate in company quality assessment and performance improvement programs.
