Then join the BHCHP One Care TeamBoston Health Care for the Homeless Program (BHCHP) is looking for a talented and compassionate Care Coordinator to provide care coordination services to individuals enrolled in our One Care program—a program for patients with Medicaid and Medicare coverage between the ages of 21-64 years of age. The Care Coordinator will work on a small, collaborative team with One Care Nurse Care Managers and other Care Coordinators as well as our medical teams to ensure that patients are being linked to needed services to improve their health and overall well-being and meet goals outlined in their care plan. You will also be responsible for spending approximately 20% of your time supporting administrative functions for the One Care team. A significant aspect of this work, however, will be direct patient engagement that will be a combination of telephonic and face to face outreach.Responsibilities:Collaborate with members of One Care team, as well as primary care teams, to support panel of medically intensive patients with complex health related social needsEngage One Care patients in services through telephonic and face to face outreach, working on-site 3 full days/week with the option for 2 days' work remote work after orientation is completedMaintain daily communication with Managed Care Organization (MCO) via web portal and phone, as neededAssist with the housing process and stability resources for patients experiencing homelessness or housing instabilityConduct outreach to shelters and patients’ homes to maintain a connection to careAssist nurse care managers and medical teams with arranging patient appointments for services, accompanying patients to those appointments, if needed, and other related activitiesPerforms referrals and service authorizations for OneCare patientsRemains flexible in duties as role expandsQualifications:Bachelor's degree preferredExperience in case management or human services is preferredComputer skills: Experience with electronic medical record documentation and navigationProficient with Microsoft Excel Spreadsheets, entering narrative and other data into a database, the use of the Internet to conduct information searchesBilingual English-Spanish preferredStrong problem solving and communication skills (written and oral)Excellent customer service skills and the ability to communicate professionally with employees and patients both on the phone, in person, and in writingEfficient, organized, detail oriented, and able to complete tasks in time sensitive mannerSelf-directed with the ability to work both independently and as part of a teamAbility to triage patients’ needs and creatively address problems as they ariseCompensation and Benefits:The compensation increases based on years of experience and ranges from $22.07 - $35.31 hourlyBHCHP full time employees are eligible for our competitive time off program, health, dental and vision insurance, 403B retirement savings plan, pre-tax MBTA pass program with 40% discount, additional compensation for demonstrated bilingual proficiency and more.
