Key ResponsibilitiesConduct in-home visits to assess client needs and develop tailored care plans.Provide training and ongoing support to family caregivers, empowering them to deliver effective care.Monitor clients’ health and coordinate with their primary care physicians to ensure continuity of care.Facilitate smooth care transitions and maintain detailed, compliant documentation.Stay up-to-date on regulations and best practices, ensuring high-quality and ethical care. What We’re Looking ForActive RN license in the state of employment.Minimum 2 years of recent experience in elder or disability care.Strong case management, care planning, and assessment skills for complex medical needs.Proficiency in MS Office and case management systems.Valid driver’s license and ability to travel locally.Dedication to compassionate, patient-centered care.
