Patient Services Coordinator - LPN Location: Boynton Beach, FL (Full‑time, 40 hours per week) Base Pay Range $45,400 – $61,300 per year (subject to experience and skills) Responsibilities Manages schedules for all patients, editing shifts for agents calling in sick and ensuring timely reassignment. Updates agent unavailability in the worker console and initiates infection control forms as needed. Serves as backup during lunch hour and busy periods, handling field staff calls and assisting with weekly case conferences. Maintains the client hospitalization log, enters coordination notes, and distributes electronic logs to office, field, and sales staff. Completes scheduling tasks as they appear on the action screen, ensuring staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is also required. Completes requested schedules for all add‑ons and applicable orders: Schedules discharge visits, OASIS collections, or recert visits following case conferences. Schedules TIF OASIS collection visits and removes remaining schedules. Reschedules declined or missed visits if appropriate. Processes reassigned and rescheduled visits. Ensures supervisory visits are scheduled. Runs all scheduling reports, including Agent Summary Report and Missed Visits Done on Paper Report. Prepares weekly agent schedules, reviews for productivity/geographic issues, and forwards schedules for Branch Director approval before distribution. Verifies visit paper notes in the scheduling console as needed. Assists with internal patient transfers between branch offices. For clinical visits, receives lab reports, assesses normality, faxes copies to doctors and case managers, routes to Medical Records, and initiates Employee/Patient Infection Reports. May perform patient visits and/or participate in on‑call rotations as required. Required Experience & Skills Licensed Professional Nurse or Licensed Vocational Nurse in the state of practice. At least 1 year of home health experience. Prior packet review or QI experience preferred. Coding certification preferred. Valid state driver’s license and automobile liability insurance. Current state licensure for employment location. Excellent communication, organizational, and multitasking skills.
