Summary:
Under the direction of the registered nurse(RN), provides nursing care in accordance with departmental andfacilitypolicies and proceduresas well as with their State Practice Act.Thelicensed practicalnurse(LPN)assists the RN withassessing, planning, implementing, and evaluatingresidentcare. TheLPNis responsible for documentation oftheir care includinginterventions, outcomes,andresident/family education. They collaborate with physicians and other members of the interdisciplinary health care team to provide continuity of care.The LPN is responsible for the direction and assignment of nursing interventions that may be performed by othersas well as care and interventions delegated to them by the RN. Must demonstrate the knowledge and skills necessary toprovide age-appropriateresidentcare and is accountable to the policies and procedures of the organizationand St. Joe'sRehabilitationand Care Center.

- Performs delegated portions of the admission assessment in accordance with specific department standards.
- Updates and documents the resident plan of care, once it is established by the RN.
- Upon admission and as appropriate, assists the RN with resident/family/others discharge planning and teaching needs.
- Delegates and assigns appropriate resident care to aides.
- Recognizes changes in resident status based on interpretation of assessments. Documents and reports findings appropriately and timely.
2. Demonstrates the skills and judgment necessary to implement medical plan of care, nursing interventions and procedures as necessary for the care of the resident in collaboration with RN.
- Demonstrates knowledge of organizational and nursing home policies and procedures for administering, verifying, and recording medications.
- Demonstrates knowledge of commonly used meds: action, dose, and side effects.
- Functions successfully in an emergency situation. Maintains professional composure and confidence during stressful situations.
- Identifies resident care problems and communicates them to RN.
- Handoff reflects current plan and status of the resident.
3. Maintains timely and accurate documentation of care provided to ensure quality care and the integration of information for use by the healthcare team.
4. Performs all responsibilities/duties required by St. Joe's Rehabilitation and Care Center LPN. Department and/or Role Specific duties include:
- Works in the capacity as a Charge Nurse- adhering to state specific board of nursing scope of practices and the organizational and facility policies and procedures.
- Meets with assigned nursing staff, as well as support personnel, in planning shifts, services, programs and activities.
- Informs nursing personnel of new admissions, expected arrival time, room assignment, etc. Ensure rooms are ready for new admissions. Greet newly admitted residents and their family and orients them to the facility.
- Reviews resident's chart for treatments, medication orders, diets, etc. as necessary.
- Assesses, evaluates, and provides ordered treatments to residents.
- Consults with providers in providing resident's care, treatment, rehabilitation, etc. as necessary.
- Ensures personnel providing direct/indirect care to residents are providing such care in accordance with the resident's care plan and wishes.
- Reviews and updates the resident's written discharge plan as needed.
- Assists in resident in restorative and rehabilitation programs. Performs range of motion as required; assists in ambulation; provides emotional support and monitors results of physical therapy activities.
- Provides preventative health care services. Makes recommendations to the resident concerning nutrition, prevention of common skin disorders, etc.
- Participates in facility surveys as requested.
- Medication Administration:
- Prepares and administers medications/treatments using the 5 rights, as ordered by the physician.
- Ensures that an adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the residents. Report needs to Nurse Manager.
- Orders prescribed medications, supplies, and equipment as necessary, and in accordance with established policies.
- Ensures that narcotic records are accurate for the shift. Immediately notifies the Nurse Manager of any discrepancies.
- Reviews medication cassettes for completeness of information, accuracy in the transcription of the physician's order, and adherence to stop order policies.
- Notifies the attending physician of automatic stop orders prior to the last dosage being administered.
- Directs and Monitors Medication Aides, "The acceptance of responsibility for observing and taking appropriate actions regarding any desired effects, side effects, interactions and contraindications associated with the medications."
5. Assumes all other duties and responsibilities as necessary.
- Demonstrates willingness to identify and/or assume activities relative to the developmental needs of the unit and the nursing department
- Demonstrates dependability/flexibility in meeting scheduling needs of the unit and facility.
- Demonstrates willingness to float to other units as needed.
- Accepts assignment as a preceptor for new staff.
- Accepts responsibility for mentoring healthcare students.
Hours will be dependent on patient census and workload. Ability and willingness to work a flexible schedule, to include after-hours and weekends as necessary.

EDUCATION:
Graduate of an accredited school of nursing required.
CERTIFICATES, LICENSES, REGISTRATIONS:
Current Licensed Practical Nurse (LPN) license or temporaryfrom the State of Nebraska required.
Basic Life Support (BLS) obtained within one full monthfrom date of hire required.
EXPERIENCE:
Previous clinical experience preferred.
SKILLS:
Language Skills - Ability to read, write, speak, and understandthe English language required.
