Physician Leader, Primary Care Base: $300k+ + bonus + $50k retention. Schedule: Mon‑Fri, no weekends or evenings. Clinical oversight of 2 APPs. Collaborative time will correlate to assigned APPs and physicians. Participate in at least one ride‑along with all collaborative APPs per quarter. Provide a complex patient visit as needed for each APP under supervision. Collaborate with clinical team members (CHWs, medical social workers, nurse navigators, care managers, patient care coordinators, etc.). Develop and participate in APP clinical development (evidence‑based care, professionalism, etc.). Lead and participate in high‑risk huddles, ensuring all patients discussed have documented plans of care that include all care‑management team members. Build a team‑based culture and work collaboratively with clinical team members. Conduct 10 chart audits per month or per state regulations, whichever is higher. Serve as a resource for clinical team members for patient‑care questions/concerns and participate in difficult conversations with patients/family members as requested by the team. Routinely conduct panel reviews with APPs. Sign DME, death certificates, and other orders as needed on behalf of APPs (as regulated by state). Frequently review quality metrics, pod‑level performance, and identify areas for improvement to achieve superior HEDIS performance. Review clinician capacity and productivity performance; partner with site medical directors to develop countermeasures and support implementation when providers are off track. Work collaboratively with the regional medical director to identify opportunities for care improvement. Assist with service inquiries, resolution of patient complaints and requests for the pod as needed. Assist pod providers in achieving higher EMR efficiency. Provide patient care to assigned panel and other patients as needed (high‑risk or complex patients, physician face‑to‑face requirements, cognitive assessments, etc.). Participate in and lead clinical education regarding accurate diagnosis, documentation, and management of chronic conditions. Assist with compliance and HR resolutions. Be responsible for discipline actions and performance reviews for direct reports. Additional duties as assigned by the regional medical director related to clinical pod performance and outcomes. Doctor of Medicine or Osteopathy. Valid driver’s license and good driving record. Active CPR certification. Active medical license. Minimum 1 year of experience with HarmonyCares Medical Group (preferred). Board certification (preferred). Compensation Posted minimum pay rate: USD $250,000.00 per year. 1 Posted maximum pay rate: USD $285,000.00 per year. Individual compensation packages are based on various factors unique to each candidate, including skill set, experience, qualifications, and other job‑related considerations.
