Role Overview We are seeking a Motivated RN to join our team in Grand Junction as a Case Manager to assist with Private Duty Nursing Cases, Skilled Nursing Visits, and new Market Growth and Development in the Western Slope. New graduates are welcome to apply. Responsibilities Provide private duty nursing care to patients in their homes to help with caseload growth until full to continue in a supervisory capacity. Provide skilled nursing visits to establish census. Administer medication, treatments, and therapies as prescribed by the physician. Monitor patient’s condition and report any changes to the healthcare team. Assist with activities of daily living and provide emotional support to patients and their families. Collaborate with other healthcare professionals to ensure optimal patient care. Maintain accurate and complete medical records using a web-based electronic medical records system. Assist with marketing and market growth development in Grand Junction and surrounding areas. Partner with talent acquisition and patient alignment. Attend career fairs, partner with local universities and attend marketing opportunities as needed. Qualifications Active RN License. Previous Home Health experience preferred. Current BLS Certification. Valid Driver’s License and Vehicle Insurance. Strong time management and prioritization skills. Excellent communication, assessment skills and attention to detail.
