The Cleveland Clinic Regional Health System includes 13 hospitals and over 180 outpatient facilities in Northeastern Ohio.We are seeking candidates interested in creating a program for heart failure at three Cleveland Clinic Regional Hospitals - Cleveland Clinic Akron Hospital, Cleveland Clinic Mercy Hospital and Cleveland Clinic Hillcrest Hospital. The focus of the position is three-fold: (1) creation of systems of care for high quality and efficient hospital care of heart failure patients, (2) population health management of outpatients with heart failure, and (3) appropriate identification and follow-up of patients needing advanced procedural treatments. Multidisciplinary coordination of care with primary care, cardiovascular specialists, and procedural teams will be a core component of the position. Electronic medical record population health analytics will also be an important aspect of the program.Cleveland Clinic Akron, Cleveland Clinic Mercy Canton and Cleveland Clinic Hillcrest Hospital are premier cardiovascular programs with high patient volumes and full cardiovascular services included interventional cardiology, electrophysiology, and cardiac surgery. The position will also coordinate care closely with Cleveland Clinic Main Campus which can provide patients access to mechanical support options and cardiac transplantation. Opportunities to round on Main Campus advanced heart failure services can be included in the position.Candidates should be BC/BE in Cardiovascular Medicine. Advanced subspecialty training in heart failure is strongly encouraged.
