* Maintain up-to-date patient records and submit timely reports in accordance with regulatory and organizational guidelines.

*Requirements:*
* Minimum 2.5 to 3 years of experience as a Physical Therapist, with a strong preference for home health experience.
* Graduated from an accredited school of Physical Therapy.
* Valid WA State PT License.
* Basic Life Support (BLS) certification from the American Heart Association (AHA).
* OASIS certification preferred but not required.
* COVID-19 vaccination card.

*Skills and Experience:*
* Proficiency in conducting patient assessments and developing effective treatment plans.
* Experience in home health care, inpatient, and outpatient settings.
* Strong communication and interpersonal skills to effectively interact with patients, families, and healthcare teams.
* Ability to work independently and manage a diverse patient caseload.
* Knowledge of home health care regulations and best practices.

*Additional Information:* This is a full-time position with a 5x8s, Day shift schedule, offering a competitive pay range of $50 to $55 per hour.
