The Hospice Physician is a key member of the hospice team that cares for the hospice patient and oversees the plan of care. This position works under the supervision of the Hospice Medical Director and serves as the Medical Director designee as needed. The Hospice Physician complements the attending provider care, acts as a medical resource to the interdisciplinary group, ensures continuity of Hospice services, and ensures appropriate measures to control patient symptoms. The Hospice Physician serves as a Hospice champion, promoting and representing the program to physicians, physician groups, discharge planners, other referral sources, community health organizations, and potential donors as appropriate. Supports, demonstrates, and promotes the principles of a respectful workplace and core corporate values in all interactions with patients, patient’s family members, other employees, vendors, or visitors. Under the supervision of the Hospice Medical Director, assures continuity and coordination of all Medical Services, ensuring the availability of medical services 24 hours a day, 7 days a week. Reviews clinical information and provides written certification that patient life expectancy is six months or less at required time frames. Participates in the development of the plan of care for patients admitted to Hospice. Acts as a liaison to physicians in the community by providing consultation and education to colleagues related to admission criteria for hospice and palliative care. Serves as a resource to the hospice interdisciplinary and attends IDT meetings required. Adheres to requirements, terms and conditions required by Medicare Conditions of Participation, accrediting body, and federal and state statutes governing the provision of services. Assists in developing and reviewing education programs for physicians, personnel, and volunteers. Participates in quality assessment performance improvement (QAPI) plans, monitoring teams, and identifies educational needs. Works with the Hospice Medical Director and Hospice Program Director to determine the impact of programs on quality of care. Assists the Hospice Medical Director in providing training regarding the medical aspects of caring for terminally ill patients to physicians, personnel, and volunteers. Qualifications Education: Currently licensed as a Doctor of Medicine or osteopathy in the state of Connecticut without restriction or subject to any disciplinary corrective action with a minimum of five years of clinical experience in medicine. Experience: Experience in Hospice or palliative care and/or training in end-of-life care; knowledge of the psychological, spiritual, and medical aspects of Hospice care. Licensure: Current license as a Doctor of Medicine or osteopathy in Connecticut with no disciplinary action, and minimum five years of clinical experience.
