Job Description
Job Title: Licensed Practical Nurse (LPN) - Full Time

Job Specialty: Long-Term Care (LTC)

Job Duration: 12 Months

Shift: 5x8.0 (Day/Evening)

Guaranteed Hours: 36-40 hours depending on shift assignment

Experience: 1 year in an LPN capacity in a Long-Term Care facility

License: LPN Licensure

Certifications: Basic Life Support (BLS)

Must-Have: Weekend and holiday availability required

Job Description:

- Provide direct and indirect skilled nursing care for assigned residents following defined policies and procedures under the supervision of a Registered Nurse.
- Maintain a collaborative relationship with nursing staff, physicians, and ancillary staff.
- Demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit.
- Assess data reflective of the patient's status to identify specific needs, providing care for adult and geriatric patients.
- Collect data relevant to residents' physical and psychosocial conditions on an ongoing basis and communicate findings to the RN and members of the Interdisciplinary Team.
- Contribute to creating a developmentally age-appropriate and culturally relevant resident care plan involving the resident/family and healthcare team.
- Recommend revisions to the care plan after evaluating resident outcomes and indicate the need for referrals to ensure comprehensive care.
- Collaborate with residents/families and members of the Interdisciplinary Team within the scope of nursing practice to ensure customer satisfaction.
- Maintain current knowledge and competency in nursing practice through continuing education and in-service programs.
- Participate in the mentoring/orientation of staff and plan delivery of care considering factors related to safety, cost, and impact.
- Provide leadership in the professional practice setting under the direction of the RN.
- Anticipate occupational exposure to bloodborne pathogens, with tasks involving potential contact with blood, body fluids, or tissues.
