Title - Cardiovascular Technician

Services Provided/Scope/Purpose: The Certified Medical Instrument Technicians (Cardiovascular Technicians) function independently as a member of the cardiovascular lab and reports to the nurse manager. The technician works with the patient care team that includes registered nurses, cardiovascular technologists, nurse practitioners, physician assistants and other providers. The Cardiovascular Technicians are responsible for assisting in the development and implementation of systems to assure the smooth and efficient flow of patients for procedures in the Cardiovascular Lab.
Cardiac procedures include but are not limited to:

Biopsies
Aortic Valve Replacements
Pericardiocentesis
Valvuloplasty
Peripheral and intracardiac ultrasound studies
Quality control of all the equipment in use
Intracoronary rotablator
Temporary intravenous pacemakers
Quality control on transfer of images and CPRS documentation using XPER Phillips System
UDI tracking
Intracoronary and ultrasound/Spectroscopy Chemogram
Intra-cardiac Ultrasound
The work requires the contractor's key personnel to record and interpret data from highly specialized x-ray, infrared, mechanical, electrical, electronic, ultrasonic, and digital computerized systems. The cardiovascular technicians must possesses knowledge of all cardiac procedures taking place in the Cardiac Catheterization Lab through knowledge of cardiac anatomy/physiology/pathology.
The Certified Medical Instrument Technicians (Cardiovascular Technicians) are required to be accountable, and understand and have knowledge of supplies (catheters, wires, balloons, stents etc.) and other equipment involved to perform these complex procedures. The Certified Medical Instrument Technicians (Cardiovascular Technicians) must independently perform the technical aspects of highly intense invasive procedure of different types of cardiology studies, hemodynamic calculations, and electrophysiology and adapt equipment to that patient's conditions for the best study/imaging quality and results.
Functions of the Certified Medical Instrument Technicians (Cardiovascular Technicians) include:
Transporting patients, preparation for the procedures such as prepping the access site, placing monitor including oxygen saturation sensor, blood pressure cuff, preparation of sterile field, draping patient following strict sterile techniques, transducer set ups, calibration and operation of complex technical equipment for the invasive cardiac procedures such as transferring images to cardiology PACs system, X-fluoroscopy equipment for coronary/peripheral angiography procedures, Intravascular Ultrasound Imaging/Pressure wire system, Rotablator, Medrad - Injector, Angio jet Thrombectomy System, Intra-Aortic Balloon Pump, Defibrillator, Bio sense Webster/BP Bard System, Valley Force Electrosurgical, Doppler, EKG machine and other cardiac equipment as required.
Applying knowledge, skills, and abilities to new software that interfaces with hospital CPRS and to upgraded Siemens digital X-ray room, where high skill technique is demanded from the operator to perform a procedure, such as assign specific protocol for different types of patients. Knowledge to transfer patient image data from server to inside the procedures room. Ability to operate the new software and apply it to that specific digital equipment.
Ensuring that the equipment is properly maintained and functioning properly. Troubleshooting and following the manufacturer's recommendations for operation of the equipment. Reporting any equipment malfunction to appropriate personnel (Ex. Biomedical Department, etc.) to ensure prompt corrective actions as necessary. Reporting work orders needed for equipment repair or breakdowns and following up.
Adhering to hospital RMB guidelines and establishes standard operating procedures (SOP).
Participating in the review and updates of Cath Lab procedures and equipment guidelines.
Operating the Phillips Allura software equipment
Searching and registering new patients according to the modality
Applying different protocol settings for the CTO procedure to decrease the X ray exposure towards the patient and the staff by adjusting the exposure of the x ray tube applying different protocol depending on the use of the room be it for Angiograrn exam, Electrophysiology study, Right Heart, PCI, Intracoronary Stent, or Pacemaker Procedure.
Operating the Client Vascular Intra-cardiac ICE Vivid ultrasound equipment, registering the patient, and choosing the appropriate transducer to obtain comprehensive real time results
Participating in new procedures such as the Chronic Total Occlusion (CTO) and applying knowledge in Cross Boss System Procedures and the Stingray System from Boston Scientific
Assisting the physician in placing collagen patches, and the Minx Vascular Devices with applying knowledge and skill towards products from Vascular Solution
Archiving ultrasound patient data images from Volcano software to the hospital server
Participating in complex radial procedures, applying KSAs on how to prep a patient for Transradial approach, and understanding the multiple supplies used to perform this procedure.
Applying KSAs to assist the interventionist in use of Arterial occlusive devices (TRB and Devices) and employing knowledge in assisting with archiving hemostasis.
Operating and maintaining Intravascular Ultrasound (IVUS) machine.
Applying knowledge of ultrasound imaging and lesion morphology
Applying abilities to measure vessel size, stenosis ration and accurately records information inside coronary artery, setting up equipment prior to procedure and handling troubleshooting with minimal supervision.
Verifying transfer of images to the Client's Cardiology system.
Applying KSAs to operate the Integrated Intravascular Ultrasound IVUS system.
Participating in the time out in each procedure to ensure patient safety and confirming patient identity using two identifiers.
Actively participating in staffing-service training and maintaining/updating own knowledge for all diagnostics and interventional equipment.
Operating and maintaining Pressure Wire system and setting up for procedures by using proper transducers/supplies to obtain hemodynamic data.
Operating digital console, normalizing pressures/calibrate equipment, and troubleshooting accordingly.
Operating and troubleshooting Angio jet Thrombectomy System, Rotablator equipment, and Shockwave during cardiac procedures.
Preparing equipment using appropriate technique for venous and blood sample collection for cardiac output analysis and assisting the physician to determine cardiac output using the nodulation techniques.
Serving as the first assistance to the attending cardiologist in the absence of a fellow physician during cardiac procedures.
Using scrubs and gowns, following infection control protocols, preparing the catheters, guide wires, stents, transducers, and manifold assembly for the procedures as well as operating the table and image tube for physician to obtain optimal angiographic studies.
Performing all exams utilizing surgical sterile techniques which requires 'scrub-in techniques”, preparation of surgical trays, preparation of contrast, supplying of catheters and guide wires, sterile assurance of the patient and equipment that may come in contact with the sterile field with the purpose of assuring that all preventive measures have been taken to prevent contamination. This is done to minimize hazards associated with invasive procedures and involves the use of sterile packs, gloves, syringes, guide wires, catheters, clean- up trays, etc. after the exam is completed.
Possessing the ability to solve standard and non-standard technical problems that the technician must coordinate with other to resolve.
Applying advanced knowledge of the theories of anatomy, physiology, and pathophysiology of the heart to calculate valve gradient and knowledge of structural valve disease showing initiative and independent judgment in resolving details inherent to infection control and safety policies/techniques with emphasis on sterile technique; the disposal of body fluids and sharps; proper attire; hand washing practices; and continuing education.
Performing patient skin preparation and draping under sterile technique prior to the procedure; opening and preparing sterile instruments and trays.
Properly wearing required sterile garment as required for the procedures according infection control policies. Disposes of the contaminated materials and other supplies used during procedure in the appropriate manner.
Applying sterile dressing to access site as needed after the procedures and immediately reporting any hematoma, bleeding, or any abnormal findings to the physician.
Attaching the cardiac monitor leads, blood pressure cuff and pulse oximetry in the appropriate positions, monitoring the patients' hemodynamic status through the procedure, and informing the physician of any changes.
Assisting and anticipating the needs of the interventional cardiologist and being continuously on the alert for changes in patient's status.
Communicating with the supply/logistics department to ensure that adequate quantities of supplies are available to meet workload demands; working in collaboration with Logistics to stock the angiography procedure rooms with necessary supplies; being knowledgeable of cardiac supplies, and assembling and preparing catheters, balloon, and stents as required for multiple cardiac procedures.
Possessing the ability to perform different techniques, adapting to equipment and accessories for best results during the procedure, and assisting with coordination and input of new products/equipment evaluation.
Participating in the orientation of patients to the laboratory environment, maintaining good rapport, providing emotional support as required during the procedure, and assisting with patient transport coordination prior to and after procedures.
Possessing knowledge and skills of emergency procedures in the Cath lab and can contacting appropriate personnel in emergency situations.
Recognizing life threatening situations and initiating appropriate emergency resuscitation measures.
Possessing the ability to identify different types of cardiac diseases, treatment modalities, and anticipating needs for specific diagnostic and interventional procedures.
Possessing knowledge of radiation safety precautions to reduce exposure to patients, staff, and self.
Wearing lead apron and film badges properly at all times to comply with hospital radiation safety regulations.
Being available on call after duty hours and weekends to provide coverage for emergency cardiac procedures as required.
Demonstrating the ability to establish interdisciplinary teamwork and leadership skills.
Demonstrating tactful verbal communication with coworkers, patients, and visitors.
Ensuring patient safety and privacy in accordance with the hospital safety standards and HIPPA.
Performing other duties as assigned to meet the service goals and mission of the department.
Certified Medical Instrument Technicians (Cardiovascular Technicians) to provide support services on site in the Cardiac Catheterization Lab
3. Qualifications:
Staff/Facility:
3.1. Certifications: The Contractor's technician(s) assigned by the Contractor to perform the services covered by this contract shall have a current certification as required in any State, Territory, or Commonwealth of the United States or the District of Columbia when services are performed onsite on Client property. Technician(s) who perform Medical Instrument Technician (Cardiovascular Technician) services must have RCIS or RCES certification.
3.2. The technician(s) must have a minimum of two (2) years current experience (within the last 6 months) in a busy cardiac catheterization lab/operating room with experience in diagnostics and therapeutic procedures focusing on coronary arteries which involves: transporting patients, preparation for the procedures such as prepping the access site, placing monitor including oxygen saturation sensor, blood pressure cuff, preparation of sterile field, draping patient following strict sterile techniques, transducer set ups, calibration and operation of complex technical equipment for the invasive cardiac procedures such as transferring images to cardiology PACs system, X-fluoroscopy equipment for coronary/peripheral angiography procedures, Intravascular Ultrasound Imaging/Pressure wire system, Rotablator, Medrad- Injector, Angio jet Thrombectomy System, Intra-Aortic Balloon Pump, Defibrillator, Bio sense Webster/BP Bard System, Valley Force.
3.3. Technical Proficiency: Technicians shall be technically proficient in the skills necessary to fulfill the government's requirements, including the ability to speak, understand, read and write English fluently.
Hours:
Facility is open 24 hours per day, 7 days a week, 365 days per year. The operating room hours are Monday through Friday 6:30 a.m.- 7:30 pm. Weekend hours are based on urgent and emergent cases.
5.1.1. Work Schedule: Contractor's personnel must be onsite at the facility in a timely manner in accordance with Rules and Regulations. Shift posted at a minimum of 3 weeks in advance. Contract key personnel will be scheduled (Monday - Friday) for the following shift:
7:30 am 4:00 pm
5.1.2. Personnel must be on-site at the facility for their assigned shift, in their scrubs and ready to work at the start of their assigned shift. The Contractor must notify the COR prior to the start of any tour when assigned key personnel are unavailable for their assigned shift.
5.1.3. Off-hours Coverage: Contractor must make personnel available on-call during all hours when the Facility clinic is closed, including evenings, weekends, and holidays.
Contract personnel will rotate with VA staff to provide off hours call coverage up to 8760 hours per option period. Off hours call coverage hours are:
Weeknights: 4:00pm – 7:30am
Weekends: 4:00pm Friday – 7:30am Monday
Call coverage may result in call back to staff the Cardiac Cath Lab at the medical center. On-call personnel must be available with 15 minutes by phone and on-site within 60 minutes.
