Develop programs to optimize successful discharge with highest function possible. Make referrals to continuation programs as necessary. Collaborate:* Client Records and Billing: Provide accurate and timely documentation of services rendered utilizing agency EMR. Demonstrate understanding and knowledge of all payer sources and authorization required. Complete and document supervisory visits with the PTA and/or HHA per regulations. * Communication: Communicate effectively with clients, families, referral sources, physicians, and the interdisciplinary team. Demonstrate exceptional verbal and written communication skills.
