Care Manager RN - Field-Based (South-Central Illinois)As a field-based Care Manager RN at CVS Health, you’ll serve dual-eligible members across South-Central Illinois, including Christian, Jefferson, Madison, and Lawrence Counties. You will coordinate care, develop individualized plans, and collaborate with providers and community resources to improve member outcomes and reduce costs.Territory AssignmentField-based role supporting members across Southern Illinois counties (Christian, Jefferson, Madison, Lawrence).Candidates should reside centrally within the region for timely access to members.Regular travel throughout the assigned territory for home visits, multidisciplinary care team meetings, and community engagement.Program OverviewProviding integrated care to dual-eligible populations through the Dual Special Needs Plan (D‑SNP FIDE).Addressing medical and social determinants of health with a compassionate, coordinated model.Position SummaryThe Care Manager RN applies a collaborative process of assessment, planning, facilitation, and advocacy to meet members’ health needs.
