Creates additional team patient care access by delivering direct patient care though in-person and virtual appointments as well as collaborating with other providers on the team to help patients with their needs between office visits.Principle ResponsibilitiesCollaborates with assigned providers within a clinic to ensure optimal care for patientsParticipates in health care teamPromotes communication within the team and other interdisciplinary health care staff.Acts as resource to members of the team.Works with primary care team for program development.Acts as liaison with outside agencies as needed.Provides clinical care for patientsCompletes comprehensive patient assessments.Determines diagnosis and treatment of illness.Orders and interprets medical diagnostics in tandem with attending physician or APC with assigned panel.Prescribes medications, treatments, therapy and medical equipment.Conducts patient education when working to manage chronic illness or wellness care.Refers for specialty care when determined.Consults with physician or more experienced APC PCP when patient needs outside of APP scope of practice, experience or diagnosis or treatment plan is unclear.Other duties as assigned.Required QualificationsMasters or PhD Nurse Practitioner or Physician Assistant programNew Grads will be consideredPreferred Qualifications 1 year Primary Care NP/PA ExperienceLicenses/CertificationsRN (for NP) State of MN and/or WI license requiredNP or PA State of MN and/or WI license/cert requiredDEA requiredBLS required within 180 daysPhysical DemandsMedium Work*:Lifting weight up to 32 lbs. occasionally (*Allina Safe Patient Moving Policy), up to 25 lbs. frequentlyPay RangePay Range: $134,550 to $153,180 per yearThe pay described reflects the starting base pay range, assuming a full-time FTE.
