We welcome talented individuals who believe in our mission, drive the organization forward, and recognize the positive impact they can bring to our communities.Position SummaryThe Program Coordinator, Maternal Child Health (MCH) Male Involvement is responsible for program coordination and administrative oversight of Westside Healthy Start male involvement activities. This position is responsible for coordinating the functions related to program implementation, data collection, and reporting. The position provides education, referrals, and support to expectant fathers and new parents in individual and group settings.Core Job ResponsibilitiesCoordinates the program development and design, implementation, and day-to-day operations at various Westside Healthy Start sitesEngage assigned or referred father/male participants in Westside Healthy Start male involvement case management curriculum at all Westside Healthy Start sites, this includes, but is not limited to, assessing needs, developing service plans, and linking to external resourcesCollects data and aids in development and maintenance of reporting tools to submit to internal and external venuesImplement and facilitate male involvement group education classes in accordance with grant requirementsProvide call back and other follow-up services to facilitate male involvementNetwork and establish external partnerships to support effective external referral for males enrolled in the program (i.e. relationships with perspective employers pertaining to resources for employment)Develop activities of interest to engage male participants in co-parenting and child development activitiesCreate and distribute monthly reports that share all activities completed and the outcomes according to grant and manager requirementsConduct regular site visits with Westside Healthy Start Community Health Specialists to ensure co-parenting activities are successful.Other duties as assigned.Requirements/PreferencesBachelor’s degree in a health-related field (social work, psychology, counseling, rehabilitation, sociology, or other human service field), required.Minimum 3 years of working in a community based or public health setting.Minimum 3 years of case management experience in a public or private social service program or health care setting.Intermediate proficiency with Microsoft Office products and database applications (i.e. Redcap, Electronic medical record).Minimum 2 years of project management experience in working with high-risk populations.ACCESS is a Network of Federally Qualified Health Centers treating patients on the frontlines of community-based health care. Depending on positionapplied/being recruited for, candidates may be required to be vaccinated against communicable diseases and provide supporting documentation proving that they are properly vaccinated,or apply forreligious and/or medicalvaccination exemption as a part of theapplicationprocess.The pay ranges provided represent the minimum to mid-range for positions.
