This role will involve collaborating closely with clients and their families to create personalized care plans and supervising the care provided by our talented team of caregivers, to ensure that care provided meets state regulations, company policies, and client needs. This part‑time role can be for as many hours a week as you are able to do, with the hours chosen by you. Key Responsibilities Conduct initial and follow‑up in‑home assessments to develop customized care plans for personal care, companionship and homemaker services. Ensure caregiver execution of care plans align with state regulations for non‑medical care by conducting periodic check‑ins and supervisory visits. Serve as a liaison between families, caregivers, and office staff to address concerns and adjust care plans. Document care plans, assessments, and caregiver evaluations according to state and company policies. Skills Critical thinking skills to assess patient needs and document appropriately. Strong interpersonal skills to build rapport with patients and their families. Ability to be responsive and organized. Qualifications Current and unrestricted Registered Nurse (RN) license in New Jersey. At least one (1) year experience as a Registered Nurse (RN).
