* Ongoing assessment of treatment needs.
* Assist in the development and ongoing monitoring and implementation of Individual Service Plan
* Work with clients to foster the development of new improved coping, problem solving, and communication skills.
* Case management functions of linking, arranging for services and referrals
* Maintain continuous contact with the identified consumer and other key stakeholders (with signed authorization to release information) in planning and service delivery. These stakeholders might include but are not limited to social workers, psychiatrists, court officials and police, teachers (if applicable) and other human service professionals
* Educate client and if applicable caregivers/family members regarding existing community services, and act as an advocate in connecting client/families to these services
* Respond to family crisis on a 24 hours, seven day a week basis
* Consult with clients, case manager, Clinical Supervisor, Office Manager, or Executive Director when clinical concerns interface with a client's inability to pay for services.
* Assures active recipient participation by providing aggressive outreach if the recipient begins to participate less than specified in treatment plan.
* Assures implementation of client’s behavior plan across all services.
* Assures development of action strategies that are consistent with the treatment plan.
* Interview client/families to assess concrete and situational needs, strengths and weaknesses, and identify problems/needs to address.
* Manages caseload by scheduling appointment, evaluating client progress in sessions and making recommendations to treatment team.
* Provides psychological skills training to individuals, groups, parents or significant others regarding emotional problems or family situations.
* Provide back-up clinical care to clients when assigned staff are unavailable.
* Share a regular rotation providing after hours on-call supervision of organization wide clinical services.
* Maintain up-to-date client records, and prepare required/necessary reports in a timely and accurate manner
* Attend court and appointments as necessary
* Establish a good working relationship with other public and private agencies/services and interface collaboratively with them

* *Planning and Community Organizing*
* Use good judgment and maintain effective boundaries when sharing information about company with community stakeholders, other providers, and members of the community.
* Attend various community meetings and report relevant information to Clinical Supervisor.
* Participate in and actively support community advisory committees.
* Participate in community needs assessments. Implement and maintain a variety of procedures to evaluate programs and services.

*KNOWLEDGE, SKILLS and ABILITIES*
* Able to implement services as planned in partnership with families and other team members.
* Have strong communication skills with diverse populations including families, team members and the public.
* Contributes to the professional reputation of the agency.
* Ability to provide skill training, monitoring, supervision, and support as indicated in treatment/support plans.
* Ability to provide therapeutic counseling activities based on current evidence based practices.
