Position Summary: The Population Health Supervisor leads the Advanced Care Primary Care staff in partnership with the Population Health Leader and Clinic Leadership. May directly supervise Ambulatory Care Managers, Registered Nurses (RN), Social Workers (SW), Licensed Practical Nurses (LPN) and/or Clinic Support Staff (CSS), ensuring high-quality care for high and rising-risk patients. Collects and interprets data that is population-based in partnership with the value-based care analytics and population health teams. Provides and directs efforts to ensure patients receive appropriate, effective, and timely healthcare services, focusing on those with chronic or complex conditions, high utilization, and high risk. This role includes care planning, patient education, medication management, care coordination and care management, aiming to improve patient outcomes, reduce costs, and enhance patient experience within the scope of practice and HealthPartners standards of care. Works with the interdisciplinary team to improve patient outcomes in alignment with value-based care initiatives with a focus on the quadruple aim. This role is comprised of 30% supervisory responsibilities and data collection and 70% direct patient care. This clinic offers an interdisciplinary team approach to patients with complex medical and social needs, with a "call us first" approach. We are looking for a Population Health RN Supervisor to join this amazing team. This role is perfect for the RN care manager looking to gain leadership experience. Work Schedule: Monday- Friday daytime hours 1.0 FTE/40 hrs weekly Required Qualifications: Education, Experience or Equivalent Combination: Graduation from an accredited program in Nursing (RN) Bachelor's degree in nursing Three years' relevant experience in nursing Licensure/ Registration/ Certification: Maintenance of a current state-certified registered nurse (RN) licensure Current BLS provider status in accordance with American Health Association (AHA) guidelines. Knowledge, Skills, and Abilities: Position requires working knowledge and basic proficiency with personal computer applications including word processing, spreadsheets, and electronic mail. Possess and maintains strong clinical skills and professional ethics as established by the Nurse Practice Act and HealthPartners. Preferred Qualifications: Previous supervisory/leadership experience strongly preferred. Care coordination, care or case management experience preferred.
