*Hours are 6:30p-6:30am with every other weekend rotation. Nursing On-Call rotation(not staffing on-call). *
* *Duties: *
* - Administer medications and treatments as prescribed by healthcare providers. Assist on the floor with cares as needed.- Monitor and record vital signs, including blood pressure, heart rate, and temperature as needed.
* Assist with cares as needed
* Monitor daily charting
* Maintain and update resident charts according to DHS 83 Regs.
* Train and coach caregivers and med passers
* Meet with families and residents for care conferences

*All other duties as assigned Qualifications:*

- Valid Licensed Practical Nurse (LPN) license in the state of Wisconsin
- Knowledge of medical terminology, CBRF regs, ECP
- Experience in Memory Care highly preferred
- Strong communication and interpersonal skills to effectively interact with residents, families, and team members.
- Ability to prioritize tasks and manage time effectively in a fast-paced healthcare environment.
- Excellent attention to detail and accuracy in documenting patient information.
- Ability to maintain confidentiality of patient information.
