We are seeking a passionate and dedicated Occupational Therapist to join our team at Canyon Creek Post Acute. As an Occupational Therapist, you will play a pivotal role in enhancing the quality of life for our patients by helping them achieve independence and overcome physical and mental challenges. Facility Type: Short Term, Rehab, 70 beds
Location: 22103 Redwood Road Castro Valley, CA 94546
Position Type: Full Time (flexible schedule)
Pay Range: $55.00 - $60.00 per Hour

SPECIFIC RESPONSIBILITIES NOT LIMITED TO:

Assess patients' functional abilities and limitations to determine appropriate treatment plans
Develop and implement individualized intervention strategies to help clients achieve their goals
Provide hands-on therapy, including task-specific exercises, adaptive equipment usage, and therapeutic activities
Collaborate with other healthcare professionals to create comprehensive treatment plans
Educate patients and their families on adaptive techniques and strategies for managing daily activities
Monitor and document patients' progress, adjusting treatment plans as needed
Stay up-to-date with the latest research and techniques in the field of occupational therapy

EDUCATION:

Graduate of program accredited by American Occupational Therapy Association (AOTA)

EXPERIENCE:

Has basic insights into geriatric care or skilled nursing home.
With Occupational Therapy Experience is preferred - new graduates must have completed a one-year clinical rotation in a Skilled Nursing setting.

LICENSES/CERTIFICATIONS:

License to practice Occupational Therapy in state of California.
BLS and CPR Certification REQUIRED
Is in good standing with all regulatory agencies and licensing boards REQUIRED

PHYSICAL REQUIREMENTS:

Physical requirements of bending, squatting, twisting, stair climbing, walking, sitting, standing, lifting, and carrying items up to 35 lbs.
Ability to use equipment and techniques to safely assist patients to transfer, move in and out of bed and ambulate.

For Support Activities:

Ability to spend 10-20% of the time sitting at a desk or terminal for data entry.
