They will collaborate with multidisciplinary providers to coordinate, manage and facilitate a comprehensive plan of care. Participate in staff quality assurance review of the Clinic's patient records, including, but not limited to, chart reviews, patient evaluation and outcome data. Plan, implement and evaluate the care of patients. Obtain consultation, collaboration and referrals as indicated. We are looking for a self-driven compassionate Nurse Practitioner to join our Behavioral Health Unit.
