The Community Care team is a collaborative and dedicated group comprising Registered Nurses (RN), Licensed Practical Nurses (LPN), Community Social Workers (CSW), and Community Health Coordinators (CHC). Together, we work closely with complex patients and their primary care providers to ensure their medical and social needs are met, allowing them to thrive at home. As the Intensive Community Care Manager, you will act as the clinical lead in managing our Community Care team. Your responsibilities will include: Conducting home and telephonic visits for high-risk patients to prevent emergency room visits or hospital readmissions. Performing initial patient assessments and developing comprehensive care plans for the LPN team. Providing clinical supervision and guidance to LPNs during patient visits. Conducting screening assessments to monitor patients’ health and educate them on self-care. Key Responsibilities include: Overseeing the implementation of care plans by providing clear instructions and education to the LPNs. Coordinating detailed case management assessments to identify outpatient needs. Collaborating with patients, families, and care team members on personalized care plans. Helping patients navigate healthcare systems and connecting them to necessary resources. Maintaining open communication with patients, families, and community providers. Fostering supportive relationships with patients to promote self-management and independence.
