Job Summary:•Provide and coordinate social services for patients from birth to over 100 years in their homes.•Advocate for patients who are working through psychological, health, family, and financial struggles.•Assess the mental and emotional stability of the patient by understanding the social and financial condition of the patient along with their family.•Coordinate and collaborate with health-care workers to provide optimal health-care services to the patient.•Utilize thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice of MGH Home Care.Key Responsibilities:•Provide and coordinate social services for patients from birth to over 100 years in their homes.•Advocate for patients who are working through psychological, health, family, and financial struggles.•Assess the mental and emotional stability of the patient by understanding the social and financial condition of the patient along with their family.•Coordinate and collaborate with health-care workers to provide optimal health-care services to the patient.•Utilize thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice of MGH Home Care.Required Qualifications:•1 year Home Care experience.•1 year of social work experience.•Current licensure in the state of Massachusetts.•Reliable, independent means of transportation to make home visits.•Must be willing to work weekends/holidays/on call in accordance with department need/rotation.•Weekend rotations are once every 6 weeks.
