Job Title: Nurse Practitioner Job Specialty: Geriatric Medicine Job Duration: 6 months Shift: Day - 5x8 - 8:00 AM - 4:30 PM Guaranteed Hours: 40 Hours per week Experience: A minimum of one year working with the frail or elderly population is strongly preferred. License: Active Indiana License required. Certifications: BLS, CDS, DEA; Board Certified by ANCC or AANP required. Must-Have: Geriatric experience; ability to work one week on and one week off for on-call coverage. Job Description: - Provide primary medical management in collaboration with physicians. - Perform assessments and collaborate with the physician as needed. - Develop and implement appropriate plans of care. - Evaluate participant physical complaints and provide appropriate treatment. - Provide teaching and education to participants and caregivers. - Function as a member of the Interdisciplinary Team. - Provide daily oversight of the clinic. - Perform rounds in contracted nursing homes. - Inform the Interdisciplinary Team of the medical condition of each participant. - Possess the ability to work independently with minimal supervision. - Demonstrate superior written and verbal communication skills. - Exhibit strong organizational and problem-solving skills. - Experience or proficiency with the Microsoft product suite and other necessary software. - Maintain a high degree of personal accountability and independent decision-making abilities. - Demonstrate adaptability to ongoing changes and work priorities. - Must be comfortable in both on-site and virtual environments.
