Director of Case Management RN or LCSW Riverside County, CA Our client is a state‑of‑the‑art, 10‑year‑old community hospital with 150 beds and a reputation for excellence. This facility is seeking an AMAZING Director of Case Management to join their team There are many reasons to consider working for this hospital and since its inception, it has received awards that are too numerous to mention but some of the highlights include: STEMI Receiving Center designation Stroke Receiving Center designation Advanced Primary Stroke Center accreditation by The Joint Commission What’s even more important: patients are treated as an integral part of the care team, encouraged to collaborate with doctors and nurses in making decisions that affect their care. Families are invited to participate in the process. Ideal Candidate Education / Training / Experience Bachelor’s degree from an accredited college or university in a related field required. Master’s degree from an accredited college or university in a related field preferred. Five (5) to seven (7) years of clinical experience in Case Management/Utilization Review required. Minimum three (3) years of leadership experience in Case Management/Utilization Review required; acute‑care experience preferred. Certifications / Licenses Current license as a Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW) in the State of California. Current Basic Life Support (BLS) or CPR certification through the American Heart Association. Must be obtained within 90 days of start date. CCM or ACM certification preferred.
