Saint Joseph PACE has an excellent full-time employment opportunity for a Board Certified Family Medicine or Internal Medicine physician with Fellowship training or Certificate of Added Qualifications in Geriatric Medicine to become Medical Director of a Program of All-Inclusive Care for the Elderly (PACE) located in Mishawaka, Indiana.
Job Description:
  • Under the supervision of the Executive Director.
  • Provides general medical direction and supervision of the medical aspects of participant care
  • Responsible for the delivery of participant care, clinical outcomes, and oversight of the Quality Assurance Performance Improvement (QAPI) program.
  • Responsible for the development and maintenance of medical policies and procedures/standards and protocols
  • Provides direction and supervision to related Primary Care Providers (PCPs) and APPs.
  • Provides leadership and guidance to the program colleagues and providers in the local community
  • Need to have a driver's license, proof of insurance, and means of transportation
  • Proficient in Microsoft Suite (Word, Excel, PowerPoint, etc.), in addition to basic knowledge of email, calendaring systems, and other software as required to perform the essential functions of the job
  • Excellent organizational, customer service, flexibility, and problem-solving skills
  • Willingness to travel within the region periodically as needed
  • Conduct history and physical on new participants and event-based and periodic reassessments. Develop medical treatment plan and provide medically necessary services in the range of locations in which participants may reside.
  • Discuss each participant's medical conditions and prognosis with him/her and their caregiver to assist them in expressing their wishes and planning their future care.
  • Collaborate in the nursing treatment program of participants.
  • Educate participants and families regarding health maintenance and their chronic medical conditions, in cooperation with nursing staff.
  • Supervise the health care and treatment program as developed by the interdisciplinary team.
  • Effective skills in physical assessment and chronic disease management for frail older adults.
  • Desire to work in a creative alternate health care setting.
  • Participates in provider on-call rotation as required.
  • Not accepting visa candidates
  • Provider must be interested in Perm (not accepting Locums)

  • Board Certified in Internal Medicine or Family Medicine
  • Board Certified in Geriatrics (Fellowship-trained) or Certificate of Added Qualifications in Geriatric Medicine (to be completed prior to start)
  • IN license (or willing to obtain)
  • Clean Malpractice
  • CDS Certification (by time of start)
  • DEA Verbal Confirmation (by time of start)
  • BLS (by time of start)
  • Minimum of 1 year experience working with the frail or elderly population
  • Previous supervisory experience
  • Does the provider require visa support - J1 or H1B?

Saint Joseph PACE Center offers coordinated clinical and rehabilitative services. It also allows participants to visit, socialize, make new friends and build meaningful relationships with doctors, nurses and other program participants. At the Saint Joseph PACE Center participants receive physician and nursing care, physical, occupational and recreational therapies, lights snacks and a hot lunch, nutritional counseling, social worker services and personal care. Medical care is provided by a PACE doctor familiar with the history, needs and preferences of each participant. br>
Trinity Health PACE is the largest provider of PACE (Program of All-Inclusive Care for the Elderly) services in the country with 13 PACE Organizations operating in nine states. PACE is designed to provide the entire continuum of care and services to frail seniors, helping them to maintain their independence in their home and community for as long as possible

Trinity Health PACE organizations are nationally recognized providing comprehensive medical, health, recreational, and social services designed specifically for each participant in an effort to promote independence at home.

PACE Organization's use an interdisciplinary team approach to provide all health, medical and social services necessary to restore and preserve the participant's level of well-being. IDT's are composed of a primary care physician, nurse, social worker, physical therapist, occupational therapist, recreational therapist or activity coordinator, dietitian, PACE center supervisor, home care liaison, health workers/aids, or their representatives, and drivers or their representatives.
