# Position#Summary Provides social work support based on initial and ongoing assessment of the patient and family needs. Counseling services are provided to assist the patient and family in minimizing the stress and problems that arise from the terminal illness, related conditions, and the dying process. Primary Position Responsibilities Provides social work care to patients and families of Hunterdon Hospice through participation in the admission process Provides counseling for patients and families to assist in minimizing the stress and problems that arise from the terminal illness, related conditions and the dying process Assesses Bereavement risk and creates a plan of care based on individual needs of family and initiates plan of care at time of patient death Actively participates in Hunterdon Hospice Quality Improvement Qualifications Minimum Education: Required: Master#s Degree in Social Work Preferred: None Minimum Years of Experience (Amount, Type and Variation): Required: Minimum of one year experience post graduate degree conferral in a health care setting Preferred: Two years of experience in hospice or palliative care License, Registry or Certification: Required: New Jersey LSW licensure Preferred: New Jersey LCSW Knowledge, Skills and/or Abilities: Required: Loss / grief issues, bereavement counseling, working with terminally / chronically ill patients Preferred: None

Position Summary
