PreferCare is seeking dedicated and compassionate Licensed Practical Nurses to join our dynamic healthcare team. In this role, you will provide exceptional patient care working with a team of other professionals and contribute to a positive and collaborative work environment. You will also have the unique opportunity to learn about and contribute to value based healthcare, an innovative approach that focuses on providing high quality care and patient outcomes.

Licensed Practical Nurses are critical members of the PreferCare clinical team, supported by our PreferCare Nurse Practitioners in providing a combination of in-home and virtual care ranging from preventive care to managing acute/chronic conditions, focusing on individuals with complex medical conditions who are at high risk for hospitalization or emergency department visits. In this role, you will be the main point of contact for our attributed patients and help connect them to the PreferCare programs, community-based services, and other supportive programs. You will conduct in-home patient visits and facilitate in-home telehealth encounters with PreferCare nurse practitioners to assess patients and help document all aspects of care, including care plans and summary notes for the clinical record. This role requires working independently and collaborating with nurse practitioners and physicians on the PreferCare team and with our partnering medical practices. This role is ideal for a Licensed Practical Nurse with extensive experience in providing patient care.
While prioritizing patient centered care is the cornerstone of your work environment, your daily routine typically combines working from your home office with visiting patients in the surrounding communities. This allows for some flexibility in your work schedule.


Essential Job Duties

To perform this job, an individual must accomplish each essential function satisfactorily, with or without reasonable accommodation.  

  • Conduct comprehensive in-home assessments of patients' health status, including physical, emotional, and social aspects. 
  • Manage a small patient panel that is sized to foster a trusting, personalized relationship with your patients. 
  • Develop and implement individualized care plans in collaboration with a multidisciplinary healthcare team. 
  • Monitor patients' conditions and report any changes to the remote Nurse Practitioner or other healthcare providers. 
  • Provide education and support to patients and their families on managing chronic conditions and preventing complications. 
  • Utilize telehealth technology to coordinate care and consult with the remote Nurse Practitioner. 
  • Document patient visits and care provided in accordance with organizational policies and regulatory requirements. 
  • Ensure continuity of care by coordinating with other healthcare providers and community resources. 
  • Participate in quality improvement initiatives and continuing education activities.

Qualifications 

  • Licensed Practical Nurse (LPN) with a valid North Carolina license. 
  • Minimum of 2 years of clinical experience, preferably in home health, emergency medicine, or a related field. 
  • Strong assessment and clinical skills with the ability to manage complex medical conditions. 
  • Proficient in using telehealth technology and electronic health records (EHR). 
  • Excellent communication and interpersonal skills, with the ability to build rapport with patients and their families. 
  • Ability to work independently and as part of a multidisciplinary team. 
  • Valid driver's license and reliable transportation. 
  • Experience in managing chronic diseases such as diabetes, heart failure, COPD, and other complex conditions preferred 
  • Certification in Basic Life Support (BLS) 
  • Familiarity with local community resources and healthcare systems preferred
