We're looking for a highly skilled Field Medical Assistant to join our High Risk team. This role involves providing exceptional patient care outside of the traditional clinic setting, leveraging technology to facilitate seamless communication between patients, families, and healthcare teams.
The Opportunity:
This full-time position will involve facilitating telehealth visits with patients in their homes, conducting clinical assessments, and coordinating care with various stakeholders. You'll be an advocate for patients, building trust and rapport through exceptional communication skills. Your passion for delivering individualized, human-centered care will drive outstanding patient experiences.
Responsibilities:
- Facilitate telehealth visits in patients' homes using technology to ensure a comprehensive view of their living situation and enable provider exams via video.
- Conduct standard patient assessments and documentation to facilitate complete visits by treating providers in the home and ensure patients understand their care plans.
- Clinical duties may include providing best-in-class telehealth setup services, performing extended scope duties such as clinician-guided physical exams, health status screenings, non-blood specimen collection, spirometry, retinopathy, EKGs, and vitals.
- Coordinate care with patients, families, and other outside care teams (specialists, home health, etc.) as needed.
- Assist patients and their families in fully participating in healthcare services and navigating community resources to support their goals.
- Participate in daily operations of the High Risk Care Team, including answering incoming phone calls, responding to emails, and facilitating transitions of care services.
- Teach patients and family members to connect with their care teams via technology.
Requirements:
- At least 1 year of experience as a Medical Assistant required.
- Experience working in home-based healthcare services or senior living care settings preferred.
- Graduated from a Medical Assistant Program accredited by CAAHEP or ABHES, or completed relevant Medical Assistant training by any branch of the armed forces or certified medical assistant certification.
- Certified Medical Assistant (MA) and Basic Life Support (BLS) preferred or ability to obtain certification within 6 months of hire required.
- Strong written and verbal communication skills; Spanish speaking a plus.
- A proven track record of persisting through change, demonstrating a forward-thinking perspective under pressure, and consistently stepping up to take action on challenges.
- A proven ability to build rapport and trust with patients or customers, while delivering individualized, human-centered customer-focused care.
- Clinical skills in performing basic vitals and clinical assessments preferred.
- Proficiency in computer technology and video platforms, such as typing, navigating the internet, meeting over Zoom, and using multiple software systems simultaneously; experience with Mac iOS and Google Suite preferred.
- Must have access to an independent means of safe and reliable transportation and the ability to travel house-to-house throughout the day; mileage reimbursed at the IRS/Federally established rate.

Compensation:** $23-$25/hour base compensation, plus additional components such as a sign-on bonus, annual performance bonus, equity grants, and other incentive compensation plans.
