Requirements : patient discharge planning hospital experience. Must have Epic and Allscripts experience. Hospice, SAR referrals, LTACHs, Acute rehabs, H.D. insurance authorizations, abuse cases, shelter admissions. Can carry a case load from 10 to 30 pts
