SITE SUPERVISOR

The Site Supervisor will supervises direct support staff in a house setting.
Qualifications
- Graduate of High School.
- Minimum of two (2) years of experience as direct support staff in home health care
- A Baccalaureate degree is preferred, but may not be required
Responsibilities
- Assists the Manager in overseeing the Agency's SCL programs
- Assists in development of policies and procedures and ensures that written documentation materials are accurate and updated regularly
- Implements evaluation of staff to ensure a high quality of care
- Participates in and conducts certain personnel evaluations
- Coordinates the documentation record review
- Hires or assists in hiring direct support staff staff
- Promotes coordination of care among all professional and paraprofessional disciplines
- Coordinates the Agency's in-service programs and ensures requirements are met
- Consults with the Manager and/or Administrator regarding ways to solve problems and maintain or improve high standards of care
- Keeps the documentation procedure resource materials current
- Promotes staff development through participation in orientation and staff education
- Participates in community activities to further the work of the Agency
- Continually evaluates the Agency's practices
- Evaluates personal job performance and departmental performance regularly
- Serves as a member of committees as requested by the Manager
Special Requirements
- Must have a car with required insurance coverage and a state driver's license or reliable transportation
Functional Abilities
- Able to communicate verbally and in writing to the extent required by the position
- Able to physically perform the duties required by the position
- Able to travel to prospective Consumers' residences
