The DHS serves as a liaison with families, hospital personnel, physicians, community organizations, and other service agencies to provide quality care to MBK Senior Living residents. The DHS is responsible for hiring, training, managing, and leading a care team to provide superior care while maintaining residents’ physical, psychological and emotional wellness, in accordance with MBK’s core values. Residents’ independence, dignity, autonomy, and purpose are integrated into the plan of care as written and directed by the DHS. Residents and their loved ones are partners in developing individualized service plans.Essential Job DutiesConduct and coordinate potential resident assessments in accordance with current rules, regulations, and community policies, including: initial assisted living resident assessment prior to move-in, comprehensive care plan at move-in; invite memory care director to participate in assessments for memory care residents; consult with regional director of health and wellness and AFG nurse case manager for complex casesDraft initial service plans for new residents; review with responsible parties and Executive DirectorEstablish service plans for each resident to promote independence and total whole-person wellness; evaluate residents through this lensConduct routinely scheduled re-assessments, change of condition assessments, and produce revised service plans; obtain signatures per policyDraft/update individualized service plans as needed; ensure personalization of care delivery; ensure best possible care and programsConduct interviews and observations of residents continually; delegate service plan processes to memory care director as appropriateAlert the Executive Director, family members and others of changes in residents’ health and the community’s ability to meet needs under licensureEnsure continuity of residents’ total wellness by coordinating health and wellness services with other resident services; work with all departments to maximize servicesAct as resource and care manager for residents and families; contact physicians for orders as neededDevelop and maintain good working relationships and communication with family and service providersDirect and assign duties to memory care director as appropriate; collaborate with wellness programming and kitchen leadership to ensure robust program offerings and nutritional wellnessParticipate in CQI, coordinate weekly interdisciplinary rounds, and oversee health/wellness databases and systems (wanderguard, eMAR, eHR, electronic check-in, pager)Collaborate on safety practices, disaster preparedness, infection control, and elopement drills; involve regional director of wellness programming for family educationInvolve MBK Together in family education offerings as appropriateNon-Essential Job DutiesPerform other duties as assigned; collaborate with AFG nurse case manager and regional director to ensure policy and regulatory complianceMaintain safe environment; comply with all regulations and company policiesDemonstrate tact and teamwork; support MBK principles and core valuesAssist Executive Director with annual budget planning and financial goalsMinimize waste and misuse of supplies and equipmentSupervisory/Management ResponsibilitiesHealth Services Assistant: 1 to 6 employeesCaregiver: 8 to 48 employees (shared with Memory Care Director and/or Director of Assisted Living)Medication Technician: 2 to 16 employees (shared with Memory Care Director and/or Director of Assisted Living)Minimum Job RequirementsCertified RN, LPN, or LVN or 2 years of full-time college attendanceCompleted required specialized nursing coursework for certification as RN, LPN or LVNMaintain license with CEUs and timely renewalPrior related supervisory/management experience in a similar settingCurrent First Aid CertificationBackground clearances as requiredHealth screening with negative TB test within last 6 months or within 7 days of employmentValid driver’s license and vehicle insurance as required by stateProficient in Microsoft Office; strong communication skills (spoken/written English)Basic math skills for personnel actions and budgetingAbility to make independent decisions and stay calm in stressful situationsProfessional demeanor with residents, families, and staffMaintain neat, organized work environment for safe care coordinationValid driver’s license in state of employment and reliable transportation for assessmentsPreferred Job RequirementsBachelor’s Degree in Nursing, Human Services, or Health Care fieldPhysical DemandsMove intermittently; ability to lift/carry up to 40+ pounds and push residents up to 250 poundsAbility to assist in moving residents when necessaryPay Rate: $104,000 - $115,000/annuallyRequirement: Valid Oregon ALF/RCF Administrator’s LicenseInspiring people and creating experiences, MBK Senior Living supports our team members and residents as we pursue becoming the senior living provider of choice in each market.EEO Statement: MBK Senior Living is an equal opportunity employer.
