To thrive as a Registered Nurse (RN) with Comfort Home Care, you'll need a versatile skill set that reflects our core values of excellence and integrity. Your role will involve conducting initial and ongoing assessments to identify clients' medical needs and their ability to communicate and learn effectively. Being detail-oriented is crucial, as you'll initiate and revise plans of care while performing OASIS assessments when applicable.
Collaboration is key; you'll coordinate services, keeping physicians and healthcare personnel updated on any changes in your clients' conditions. Strong communication skills will help you engage with patients and families, ensuring that care plans are tailored to their needs. Additionally, the ability to document clinical and progress notes accurately and submit paperwork on time is essential in maintaining our high standards of customer-centricity and safety.

Knowledge and skills required for the position are:
RN provides initial and ongoing assessments to identify the client's medical needs level of functioning and ability to communicate and learn.
RN initiates the plan of care necessary revisions and performs OASIS assessments at appropriate time points when applicable.
RN Initiates and performs appropriate nursing interventions.
RN coordinates services informs the physician and other personnel of changes in the client's condition and needs.
RN collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
RN prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
