This leadership role focuses on continual improvement of safety, effectiveness, efficiency, patient‑centeredness, equity, and timeliness of behavioral and medical health services. The Director oversees all licensing and accreditation processes to secure continuous certification and endorsement from agencies such as CARF and NARR.Essential Job DutiesProvide direct administrative supervision and oversight to the Quality Specialist teamUtilize clinical and operational data to identify quality issues, improvement opportunities, and monitor progressManage patient and referent satisfaction surveys, engagement/retention outcomes, incident reports, grievances, and internal chart auditsDevelop and implement the annual Performance Measurement Plan with the Senior Director of Operations & Compliance and Executive TeamCollaborate in creating leadership dashboards and report findings to the Executive Team, Quality Improvement Committee, and stakeholdersOversee patient rights, grievance procedures, incident reports, and the Health & Safety CommitteeChair the Quality Management Committee and drive engagement of QM team membersDevelop and maintain the Annual Quality Improvement Plan aligned with strategic goals, accreditation standards, and state regulationsLead data collection, analysis, and reporting for quality and patient safety initiativesCreate, modify, and review policies and procedures with the Senior Director of Operations & ComplianceAdvocate for and implement Continuous Quality Improvement (CQI) principles across the organizationCollect and analyze data on patient records, utilization management, and medication servicesEvaluate the effectiveness of the QM Program annually and as neededDevelop and present the Annual Quality Improvement Report addressing high‑risk, high‑volume, or high‑cost areasCommunicate QM activities and findings to the CEO and Board of Directors as requestedOversee licensing and accreditation processes, ensuring continuous certification and compliance with corrective action plansServe as liaison for licensing, accreditation, and credentialing agenciesPrepare for and host site reviews, ensuring positive results with minimal findingsAttend community‑based quality management meetings and perform other related duties as requiredAdditional Job ResponsibilitiesAttend staff meetings as scheduledComplete additional trainings as assigned in a timely mannerLead special projects and other duties as assignedParticipate in ongoing supervision and consultationProvide leadership that supports agency values of honesty, empathy, respect, inclusion, and unityProactively identify barriers to effective medication administration systems and work with residential and medical staff to address themMaintain responsive, best‑practice quality treatment servicesOversee operational aspects of clinical tasks and roles, providing strategic advice and direction to functionalize the service delivery modelSupervisory FunctionsPosition includes supervisory duties.QualificationsEducation and ExperienceBachelor’s Degree in health or human service fields (hospital administration, public health, allied health, etc.) requiredMaster’s Degree preferred; may substitute for two (2) years of experienceMinimum of 4 years of experience with multidisciplinary groups focused on service delivery and/or quality improvement in healthcareAt least 2 years of leadership experience managing teams or programsMinimum of 3 years supervisory or management experience requiredExperience in healthcare quality management and data‑driven decision‑makingProven ability to analyze and interpret aggregate data and produce actionable recommendationsExperience preparing reports and documentation for service delivery or quality assuranceKnowledge, Skills and AbilitiesStrong knowledge of CQI principles (Six Sigma, Lean)Strong analytical and problem‑solving abilities; understanding of regulatory, licensure, and risk management standardsExperience with project management, clinical data collection, and process improvementKnowledge of risk management, infection prevention, and patient safetyExcellent leadership, interpersonal, and communication skillsAbility to train, facilitate, and motivate cross‑functional teamsProficient in Microsoft Office Suite and data analysis toolsHighly organized with strong attention to detail and ability to manage multiple projectsPositive attitude with creativity, initiative, and enthusiasmValid driver’s license, clean driving record (past 3 years), and current auto insuranceAbility to travel between Fora Health sites and attend external meetings as neededSchedule may occasionally include evenings and weekendsOther RequirementsEmployees with a history of substance use disorder require certification of a minimum of two years of sobriety for this positionAbility to pass pre‑employment or for‑cause drug testsRegular connection to work via cell phone to answer questions and respond to internal and external contactsWorking ConditionsProlonged use of computerOn‑call availability to respond to emergency situations during off‑duty hours (including via cell phone)Indoor office environment with infrequent travel between sites or to special eventsMay occasionally require extended hours including evenings or weekendsFast‑paced, high‑volume work environmentPossible exposure to communicable diseases (COVID, tuberculosis, hepatitis, HIV)Possible exposure to upset, angry, severely traumatized, disabled or emotionally disturbed adults, adolescents, and familiesExposure to potentially hazardous cleaning chemicals;
