We strive to provide an inclusive environment that welcomes and values the diversity of the people we employ and serve.Job Summary: The Mobile Health Program (MHP) facilitates access to medical, dental, behavioral health, and social care for patients with the highest barriers to receiving care by taking services into the community. The Mobile Health Program Manager (MHPM) works closely with the Virginia Garcia Memorial Health Clinic (VGMHC) Community Outreach program, collaborates with community agencies, and partners with the dental, medical, and pharmacy teams to assure access to equitable, culturally appropriate, patient-centered services. The MHPM is responsible for the overall management, oversight, and coordination of the MHP to ensure an effective model of mobile healthcare delivery that reaches populations with barriers to clinical care access. This position regularly travels between clinical and mobile sites and requires a valid driver’s license and a vehicle. Role includes some weekend and late evening shifts.The Mobile Health Program includes Mobile Primary Care, the Farmworker Outreach Program (FOP), Center for Addictions Triage and Treatment (CATT), Low Acuity Transitional Services (LATS/Medical Respite), School-Based Health Center (SBHC) mobile integration, and Behavioral Health expansion. The Program Manager provides oversight and coordination of these programs and activities to ensure equitable, patient-centered care delivery across communities with barriers to access.Essential Duties And ResponsibilitiesProgram ManagementUnder the direction of VGMHC leadership, develop and oversee a mobile health program that aligns with Virginia Garcia’s mission and strategic goals.Implement strategic plan for the MHP, including monitoring key deliverables in accordance with health center goals and contractual agreements.Partner with the Community Outreach Manager and internal and external stakeholders to identify underserved communities and populations that present opportunities for program services.Coordinate with the Community Outreach Manager and clinical and operational leadership to identify barriers in service provision and develop strategies that address these barriers.Provide oversight of LATS/Medical Respite program including nurturing partnerships with homeless service providers and county partners, building bridges that ensure excellence in service delivery.Oversee the mobile team services in the VGMHC clinic at the CATT (Center for Addictions, Triage and Treatment), ensuring effective services in coordination with other service providers.Lead and coordinate the Farmworker Outreach Program (FOP) in collaboration with Community Outreach Manager, SBHC Program Manager, clinical teams, and outreach staff.Collaborate with Behavioral Health leadership to expand access to mental health and substance use services through mobile and respite models.Develop, implement, and monitor outcome dashboards and population health data to evaluate program impact.Maintain active partnerships with Washington and Yamhill County Health Departments and community-based organizations to expand service reach and collaborative impact.Partner with business services, grants, and finance teams to ensure program models are financially viable and sustainable; collaborate with EHR and Billing department to ensure effective revenue cycle management.Collaborate with the VGMHC Public Grants Officer, community partners, and the Community Outreach Manager to conduct ongoing needs assessment and direct planning and implementation of Mobile Health Program activities including vaccine and testing events, community-based public health programs, primary care, clinical pharmacy, and dental care for vulnerable populations.Collaborate with VGMHC operational leadership, data team, and Public Grants Officer to write and submit reports to federal, state, county, and private funders, including preparing and making presentations on data and program activities to the Board and Executive leadership as needed.Work with VGMHC data and evaluation teams to strengthen grant reporting, health equity analysis, and program outcomes tracking.Operational LeadershipProvide direct supervision for Mobile Health Program Supervisor (MHPS) including regular check-ins and annual performance reviews, as well as supporting orientation, training plans, scheduling and assignments, and compliance with VG culture and regulatory requirements for entire mobile team including providers.Collaborate with MHPS to plan and manage staffing of mobile unit events, including medical and dental providers, nursing, outreach, patient services, dental staff, and drivers, in consultation with managers of supporting department and service lines and the volunteer program. This includes coordination with SBHC leadership on shared staffing, outreach, and event coverage.Work with MHPS to manage staffing and logistics for mobile health events, LATS/respite care, CATT activities, and FOP outreach services.Actively participate in mobile program events and activities supporting clinical services and operational logistics when needed.Serve as a contact and liaison between the local public health authorities (LPHA), community partners, and VGMHC program, department, and service line leadership.Collaborate with VGMHC Communications on planning, marketing, advertising, and product design for mobile programs and events.Coordinate with Quality Assurance team to conduct and oversee implementation and reporting of community and patient experience surveys.Ensure compliance with health regulations, program standards, and privacy laws while identifying opportunities for improvement.Oversee behavioral health integration into mobile workflows, including warm hand-offs, telehealth, and community-based support.Ensure compliance with safety, CLIA, VFC, and other regulatory requirements.Guide workflow development that supports high-quality, patient-centered service delivery in environments that may change daily and are fast-paced, flexible, and dynamic.Work with the VGMHC clinical training team, EHR support team, and primary care sites to assure on-going education, direct support, and mentoring of MHP staff as well as training for cross-coverage at primary care sites.Ensure that all mobile units and equipment are in full compliance with safety and other regulatory mandates in partnership with Facilities.Participate in budget development, monitor budget, and work with the Finance and Foundation departments to ensure accurate tracking, documentation and compliance with funding regulations.Oversee compliance with CLIA and VFC program requirements in conjunction with the VGMHC Lab and Vaccine Program Manager and the clinical training team.Oversight of purchasing and stewardship of office and medical supplies for mobile units and events.Coordinate and facilitate huddles and department meetings and represent the program through consistent participation in VGMHC leadership meetings and huddles, escalating issues from MHP staff and disseminating relevant information to employees.Actively engage in VGMHC initiatives, projects and workgroups as assigned.Work with Epic trainers, Registration and Interpretation Services Program Manager, and the Billing department to assure accurate registration of patients, address billing error reports; track and correct encounters in EPIC error pools and identify possible training needs.Investigate patient, provider and staff complaints and develop appropriate action plans.Oversee and ensure adherence to policies and procedures.Perform other duties as assigned.HIPAA RequirementsThe MHPM has access to PHI in the course of carrying out their duties.
