Regional Clinical RN — Assisted Living, Full‑Time North Shore Health is seeking a Regional Clinical RN to provide clinical oversight across multiple assisted living communities in Wisconsin. The role ensures compliance with Wisconsin Department of Health Services (DHS) regulations—specifically DHS Chapter 83 and Chapter 89—guiding clinical teams, conducting comprehensive assessments, developing individual service plans, and delegating nursing tasks per the Wisconsin Nurse Practice Act. Clinical Assessment and Care Coordination Promotes quality of care and services for residents in assigned communities, including their physical, emotional, and social well‑being. Performs and/or reviews initial and ongoing comprehensive assessments for residents. Develops, implements, and updates individualized service plans (ISPs) based on assessments and changing resident conditions. Attends and contributes to interdisciplinary care conferences. Conducts and documents assessments per DHS guidelines such as self‑administration of medication, psychotropic medication reviews, pain assessments, fall risk, and other community‑assigned assessments. Ensures written physician orders are signed, processed, and implemented in each assigned community. Documents observations, concerns, changes in resident conditions, incidents, and/or accidents, ensuring follow‑up until resolution. Serves as liaison between residents, resident representatives, and physicians/health‑care providers regarding resident health‑care issues. Coordinates care for residents returning from hospitals or skilled nursing, ensuring appropriate transitions and follow‑ups. Monitors residents’ health status and coordinates care with physicians, home‑health agencies, hospice providers, and other external partners. Identifies and responds promptly to changes in residents’ health conditions, ensuring timely communication with physicians and resident representatives. Participates in resident admission and discharge planning. Clinical Excellence Maintains up‑to‑date knowledge of current standards and best practices in assisted living care. Collaborates with clinical leadership to develop and implement clinical protocols, policies, and procedures that ensure compliance with applicable laws and Wisconsin state guidelines. Designs and oversees an audit process to review nursing assessments and the development of ISPs across assigned assisted living communities. Maintains oversight of the infection control program, including monitoring employee health and related protocols. Remains accessible and available via cell phone and serves as ‘on call’ for clinical emergencies, resident changes in condition, and support of staff performing delegated nursing tasks. Ensures adherence to all applicable assisted‑living licensure regulations. Monitors the timeliness and quality of nursing assessments for residents and provides clinical support when needed. Medication Management Provides oversight of medication management systems to ensure safe and effective dispensing. Reviews medication administration system and medication documentation at least monthly in each assigned community. Ensures that MAR (Medication Administration Records) is accurate and complete. Ensures the medication cart is orderly and that all medications are properly packaged and stored. Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, and other follow‑up with staff. Validates that medication disposal policy is followed at each assigned community. Leadership, Delegation, and Staff Development Collaborates with senior leadership and community Executive Directors to promote alignment of nursing practices with organizational goals and standards. Delegates nursing tasks appropriately per the Wisconsin Nurse Practice Act and supervises delegated tasks to ensure safe and effective care. Ensures proper training, competency validation, and ongoing support for direct care staff. Trains and supports facility RNs, LPNs, and care staff on clinical protocols and regulatory requirements. Conducts in‑service training on topics such as infection control, medication management, emergency response, and chronic disease management. Regulatory Compliance and Oversight Ensures facility compliance with Wisconsin DHS Chapter 83 (CBRF) and DHS Chapter 89 (RCAC) regulations. Monitors and assists with incident reporting, follow‑up, and mitigation strategies. Prepares for and participates in the regulatory survey process. Participates in quality improvement initiatives to enhance resident clinical outcomes, quality of life, and operational efficiency. Association Involvement & Management Works with community leadership and senior leadership to manage participation in WiCAL’s PEAL program (Performance Excellence in Assisted Living) and Wisconsin Coalition for Collaborative Excellence in Assisted Living (WCCEAL) involvement. Monitors and encourages participation of assigned communities in the Wisconsin Board on Aging and Long‑Term Care (WI BAL) forum meetings. Shares relevant information and insights from WI BAL forums with other team members as appropriate. Collaboration and Support Works closely with Executive Directors, care teams, families, and external providers to coordinate resident care. Works closely with Regional Operations, Executive Directors, and Support Center leadership to support high‑quality, efficient service delivery. Monitors resident and family concerns to validate prompt and effective resolution. Provides input and/or helps perform annual evaluations for nursing and direct care staff. Additional Responsibilities Know and follow North Shore Health rules, dress, hygiene, and smoking policies. Demonstrate proper use of equipment; report equipment needs or repairs. Use required protective equipment (as needed during center visits). Follow infection control standards, policies, and procedures. Know Resident Rights and help residents exercise and protect their rights. Maintain confidentiality of resident/patient and employee information. Follow and adhere to North Shore Health’s policies and procedures implementing HIPAA requirements for the privacy and security of protected health information; use and/or disclose only the minimum amount of Protected Health Information necessary to complete assigned tasks. Report all suspected violations of the company’s HIPAA policies or procedures to North Shore Health Compliance Department. Work Environment / Physical Requirements Responsibilities may involve significant travel and physical activities including standing, occasional lifting (up to 50 pounds unassisted), bending, stooping, pushing, pulling, and twisting. Employees visiting centers may be required to provide lifting and transfer assistance to residents, sometimes using a lifting device or assistance from other staff. Required Education and Experience Active Registered Nurse (RN) licensed in Wisconsin or a valid Multi‑State license under the Nurse Licensure Compact (NLC) with Wisconsin as the primary state of residence. Demonstrated nursing management experience in a resident/patient care setting. Experience working with older adults in senior living, long‑term care, home health, or other health‑care setting preferred; strong desire to engage with residents with dementia is a plus. Ability to adapt to a flexible work schedule and occasional overnight travel. Team‑oriented with strong communication and interpersonal skills. Empathy for the challenges experienced by residents, families, and team members and strong problem‑solving skills. Strong computer skills and ability to interact with a variety of electronic devices. Must hold and maintain current CPR certification. Previous experience in an assisted living setting. Demonstrated success leading a fun and productive work environment. Preferred Education and Experience Three or more years of management experience preferred.
