Family Support Navigator

Part-Time, 4 hour shifts, weekday evenings and weekends, 9 AM to 5 PM

General Description:

Provide outreach, engagement, support, care coordination, assessment, and person-centered treatment planning for CBHC enrolled youth and their families. The Family Support Navigator engages with youth and their families to integrate and coordinate care, support care transitions and support delivery of treatment.. They will actively support and assist youth in care to experience success and satisfaction in their lives by improving their quality of life by modeling, being an active listener, and sharing the reality of wellness and recovery. The Family Support Navigator will promote person centered self determination and decision making through the use of their own recovery stories and lived experience.

Essential Job Functions

  • Support youth to identify and develop goals
  • Model for hope and self determination
  • Use a strength based approach
  • Use personal experiences to provide pathways to recovery
  • Assist youth with goal setting, problem solving, skill building
  • Assist youth in connecting to supports i.e. community based services, activities, natural supports
  • Communicate a sense of hopefulness and respect
  • Provide encouragement and support
  • Participate in treatment planning meetings
  • Develop and maintain relationships with youth, families, community providers, resources, program staff and other links to the community
  • Attend trainings and groups that support recovery and wellness services within the agency and through other recovery and wellness trainings by SAMHSA, RLC, MPower etc.
  • Promote recovery based model services to youth through role modeling of successful recovery strategies and skills sets while promoting independence and self advocacy

Administrative functions

  • Complete required note keeping, regarding the development and progress of the group, activity or workshop
  • Represent the agency in a professional manner at all times
  • Complete all required paperwork in a timely manner
  • Participate in all relevant staff meetings, supervisions and required trainings
  • Report any incident(s) regarding accidents, injuries, and unusual events to the Program Supervisor and/or Program Director
  • Follow all agency policies and procedures
  • Maintain ethical boundaries
  • Flexibly respond to program scheduling needs and assist other programs when assigned

Other Requirements

  • Ability to pass Criminal Offense Record Inquiry (CORI) check
  • Valid drivers license and ability to use a personally insured vehicle for agency business

Qualifications

  • GED/HS Diploma required
  • Minimum of 6 months to 1 year of personal recovery with lived experience
  • Minimum of 1 year of recovery/sobriety for person living with alcohol/substance issues
  • Knowledge of community resources

Skills

  • Individual with lived experience willing to share positive recovery story for the purpose of promoting recovery and wellness for youth and to support the mission of recovery principles
  • Must have a strong personal recovery skills set
  • Problem solving skills and ability to suggest options and alternatives
  • Support the belief that youth with lived experience can and do recover
  • Strong listening, oral and presentation (communication) skills

Working Conditions

  • Works in various places in the community and at other Brien Center programs/sites

Physical Conditions

  • Manuel & visual dexterity: correctable
  • Able to work or travel inside and outside as needed in varying weather conditions
  • Ability to work in various settings (home, agency, school, community, etc.)
  • Push, pull, stand, reach overhead and twist
  • Able to bend at knees, waist and lower self to the ground or floor without assistance
  • Able to physically climb and descend stairs without assistance
  • Reasonable accommodations may be made to enable individuals with disabilities to perform essential tasks
