Overview of the Position:

The Staff Nurse is an essential member of the patient center medical home team. Responsible for managing the individualized patient care, by promoting and restoring patient's health through the nursing process; collaborating with provider and multi-disciplinary team members; provides physical and psychological support.

Education and Experience:

* Diploma in Nursing; minimum requirements or Associates in Nursing
* Licensed by the State Board of Nursing
* BLS required
* Two year medical office experience preferred

* Bilingual in English/Spanish preferred
* Familiar with medication names and usage
* Medical terminology
* At least two years of experience in a physician's office
* Excellent interpersonal skills
* Detailed oriented
* Ability to multitask
* Compassion for patients
* Ability to administer a variety of treatments and medications as directed
* Capable of adapting to computer software

Supervisory Responsibilities:

* This position has no direct reports

Essential Job Functions:

These functions will be performed by the Staff Nurse and will be the responsibility of the incumbent working with the patients, medical providers, clinical staff and non-clinical staff to ensure excellence in service for the patient.

* Assess the needs of patients based on data collected through history observation, physical exam, telephone triage, and analysis or diagnostic data
* Performs direct patient care: triage, assessment, procedures, treatment, patient education, and follow up instructions.
* Assist patients and providers in obtaining medicinal prior authorizations when required
* Verifies the patient understands the purpose of the visit.
* Providers preventative Care information matching the patients age, gender, and health status
* Maintain knowledge of the principles of the skills needed for practical nursing to provide patient care and treatment
* Knowledge of examination, diagnostic and treatment room procedures
* Ability to interpret, adapt, and apply guidelines and procedures
* Basic Critical Care and Chronic Illness Management
* Required to participate in training opportunities
* Helps promote office efficiency and encounter volumes
* Ability to understand and administer vaccines and immunizations to pediatric and/or adult patients
* Responsible for daily clinical workflow which includes but not limited to: Referral management, telephone encounters, lab results, medication refills, lab Q&A, compliance with sample medication logs
* Willingness to learn about AIDS Service Organizations and Community Health Centers
* Assist in training new nurses to the organization in the standard working procedures of HopeHealth
* All other duties assigned

Physical Requirements:

Hearing: Adequate to perform job duties in person and over the telephone. Speaking: Must be able to communicate clearly to patients in person and over the telephone. Vision: Visual acuity adequately to perform job duties, including reading information from printed sources and computer screens. Other: Requires occasional lifting and carrying items weighing up to 10 pounds unassisted in normal daily activities and up to 30 lbs. for other required work activities. Additionally, requires frequent bending, reaching, and repetitive hand movements (especially if keyboarding and writing), standing, walking, squatting, and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular work shift.
