Seeking a Locums Experienced Family Medicine Nurse Practitioner or Physician Assistant to join our primary care team in Everett, WA. The successful candidate will provide evidence-based primary care, develop patient care plans, and ensure comprehensive care within the medical home model while delivering clinical leadership. In this role, you will provide evidence-based primary care to 10-15 patients daily, addressing both chronic and acute health needs while emphasizing preventive care. You will collaborate closely with patients and a multidisciplinary care team to create individualized, biopsychosocial care plans tailored to each patient's unique needs. Responsibilities include delivering comprehensive care aligned with the medical home model—this involves taking detailed patient histories, conducting physical exams, performing diagnostics, and prescribing medications, with the added convenience of onsite lab and pharmacy services. You will also assess the health needs of the patient population, promoting preventive evaluations, health coaching, and other wellness initiatives. Additionally, you will contribute to clinical leadership, supporting communication, process improvements, and adaptation to change, while participating in an after-hours coverage rotation with fellow providers. This is a part-time schedule of mandatory Saturdays and 2 other weekdays. Requirements: Board Certification as a Nurse Practitioner (NP) with a background in primary care, family medicine, or internal medicine. Current, unrestricted medical license and DEA number. 3-5 years of primary care experience. 2 years of case management, focusing on health prevention and population health. EMR systems experience, preferably NextGen. Coding and documentation experience, including Medicare Advantage and HCC.
