RN, Hospice Clinical Manager, Lakeshore

We are growing And we are seeking a compassionate RN who has a passion for hospice nursing and leadership to join our team of Hospice Clinical Managers in leading our Faith Hospice care team professionals in the community.

What will you do?

Oversee and supervise an interdisciplinary hospice team of professionals
Direct and participate in the weekly hospice interdisciplinary group meetings
Oversee and supervise the day-to-day activities assuring quality patient care is provided on a 24-hour, 7-day basis
Participate in the Manager on-call rotation with the other hospice managers

What is needed for success in this role?

Licensure as an RN in the State of Michigan
At least 1-2 years of nursing management experience in a skilled nursing or med/surg setting
Prior hospice nursing experience is highly preferred
Strong computer skills to chart in the EMR are essential

Since 1995, Faith Hospice has been providing comfort care to those with life-limiting illness or condition as well as support to their families. Our doctors, nurses, social workers, chaplains, hospice aides, and volunteers service patients in a variety of settings. Faith Hospice provides care "wherever our patient puts his/her head down to sleep at night" including patient homes, assisted living or skilled nursing facilities, and hospitals. We also provide end-of-life care at Trillium Woods, our unique hospice residence in Byron Center. Hospice nursing requires a special gift of caring for those patients and families at such a critical time.
