JOB SUMMARY:

In collaboration with a supervising physician, the Nurse Practitioner expands the individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions for our transitional care/palliative care organization. Transitional care is a combination of disease modification, primary care and management of distressful symptoms in individuals with severe illness -- recently discharged from either an acute care facility (hospital), post-acute care facility (SNF) or long-term care facility.

Knowledge/Skills/Abilities:

The Home Visit Nurse Practitioner must be able to perform each essential duty satisfactorily. The requirements listed are representative of the knowledge, skill, and/or ability required.

  1. Make a visit to the patients home within the first 48 hours of discharge from the hospital or the first 72 hour of referral.
  2. Make follow up visits to the patients home.
  3. Medication reconciliation
  4. Transition to adjuvant pain treatment
  5. Manage symptoms, including schedule Rx for the first week, nausea, dyspnea, mood
  6. Bowel/bladder management (cathartics, depends, etc)
  7. Administer injectables, breathing treatments, remove lines and any other medical treatment necessary deemed by provider and or medical director
  8. Maintains communication with the primary care provider
  9. Provides continuity across time and space of multiple care levels
  10. Coordinates and maintains continuity with secondary services
  11. Refers to secondary providers after discussing with the primary care provider and or chief physician or other designated by chief physician for difficult and/or complex management services


Essential Duties and Responsibilities:

  • Nurse Practitioner specializing in Advanced Practice obtained prior to the hire date
  • DEA
  • Master's level Degree or MSN from an accredited program approved by the State Board of Nursing program
  • Minimum of 3 -- 5 years of clinical nursing experience and preferable background in EMT or Paramedic
  • 1-2 years of Nurse Practitioner experience preferred with inpatient hospital experience or skilled nursing experience
