**Description**

***Description of Primary Responsibilities:***

* Responsible for the overall transportation and safety of patients to and from the health centers.

* Adheres to the Preventive Maintenance Schedule for the transportation vehicles.

* Maintains transportation safety standards and daily cleanliness of transportation vehicles.

* Performs housekeeping duties when not transporting patients.

***Professional & Technical Knowledge:***

* Must possess a high school diploma or General Educational Development (GED) certificate.

* Job duties require specific knowledge of housekeeping or transportation processes and practices, typically learned on the job, or which may include a series of training sessions that would comprise a few weeks if done consecutively.

***Licenses & Certifications:***

Must have a valid Class A or B Drivers License with Passenger Endorsements if driving a 12 or more passenger vehicle, and a clean DMV record that allows employee to be eligible to be insured to transport patients by FHCNs automobile insurance carrier. In lieu of a Class A or B Drivers License, may have a valid Class C Drivers License to drive an 8 passenger or less vehicle.
