**Lead Primary Care Physician**

**Job Description**
As a Lead Primary Care Physician, you will be responsible for providing high-quality patient care, leading a team of healthcare professionals, and driving clinical outcomes. You will work closely with our team to develop and implement care plans, manage patient panels, and ensure seamless transitions of care.
**Key Responsibilities**
• Provide high-quality patient care, including assessment, diagnosis, treatment, and management of acute and chronic health needs • Lead a team of healthcare professionals, including care promoters, care facilitators, and care coordinators • Develop and implement care plans, manage patient panels, and ensure seamless transitions of care • Participate in quality improvement initiatives, including data analysis and process improvement • Collaborate with other healthcare professionals to ensure comprehensive patient care • Stay up-to-date with evidence-based medicine and best practices in primary care
**Requirements**
• MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty • Current, active MD licensure in the state of employment • Minimum of 1-year clinical experience in geriatric, adult, or family practice setting • Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty • Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross
